An older parent and an adult child go over a page together at the kitchen table.

Which medicines were taken

If you or a helper hands your parent their medicines, a simple record stops the guessing about whether tonight’s dose was taken.

Sheet preview: Which medicines were taken

How to use it

  1. If you help with doses, start one sheet for one medicine.
  2. Write what actually happened, not what was planned. If your parent tells you, write “Parent says taken.”
  3. A blank never means a dose is due, so never give extra to fill a gap.
  4. Ask the pharmacist about a missed or unclear dose.

When more than one person helps with medicines

When helpers share medicine tasks, it is easy to lose track of what was given. This record shows the next person what was taken and when. Use one sheet for each medicine: copy its checked directions onto the top line, then use one row for each dose.

Record only what you can confirm: taken, with the real date and time; not taken, and why; or not sure. Write the amount taken and your name. For a medicine used only as needed, copy its limits onto the top line and note why it was used. Mark a dose taken only after it happens.

A blank does not mean a dose is due or was missed. If a dose is missed or unclear, ask your parent or the last helper, then the pharmacist. Never double a dose or give extra medicine to fill a blank. If too much may have been taken, call Poison Help at 800-222-1222. If your parent cannot wake or has trouble breathing, call 911. Keep the record with the medicine list.
