How to use it
- With your parent, gather the labels and latest written directions.
- Copy the name and strength from each label. Write how much to take and when.
- Add vitamins, drops, creams, and drugs used only now and then. Use extra pages if you need them.
- Keep the list with the care papers for visits.
Quick care note
If the label and written directions do not match, ask the pharmacist. Do not change a dose from this sheet.
Keeping a medicine list you can trust
Your parent may manage medicines on their own. If they want help, find the hard part first: reading labels, opening packs, getting refills, or keeping track. Keep what works, and agree on your part before you make a list.
Gather the bottles, boxes, and latest care papers, including drops, creams, vitamins, and anything bought without a prescription. Check each item with a pharmacist, the medicine expert at the drugstore, and ask which directions are current. Then copy the checked directions onto the list: how much, how to use it, and when, plus any food rules and start or stop dates.
Strength tells you what is in each pill; the dose tells you how much to use. Write “ask” if you are unsure. Bring the list to every visit, date each new version, and mark old copies “OLD” so helpers use the right one. Ask before splitting or crushing pills, and never stop a medicine or change a dose on your own.
