A ride there and home
A missed ride home can turn a short appointment into a long afternoon.

How to use it
- For your parent’s next appointment, confirm the ride there and the ride home.
- The sheet has room for the address and entrance, pickup times, who’s driving each way, how the fare gets paid, and a backup if a ride falls through.
- Ask what help comes with the ride: your parent may need a hand at the door or with a walker.
Booking the whole trip, not just the ride there
Getting to a visit is only part of the trip. Use the visit notice for the address and arrival time, then call the driver or service and confirm each part: the pickup time and exact place, the ride home, and the price.
Ask what help comes with the ride: help to the car, getting in, room for a walker or chair, and whether someone can go along. Write the answers and read them back, and check that the ride is confirmed, not just requested.
Tell your parent and anyone going with them, and put the trip on their calendar. Note a backup ride in case one falls through. For local ride options in the U.S., call Eldercare Locator at 800-677-1116.
