How to use it
- Before the visit, ask your parent what they most want to know. Write that question on the sheet.
- During the visit, jot down the answer. If it is unclear, ask, “Could you say that another way?”
- Before you leave, check what happens next and who to call about test results.
Getting more out of a doctor visit
If you are going to a visit with your parent, first ask what help they want. They may want a ride, someone to take notes, you in the room, or some time alone with the doctor.
Before you go, choose the top question together and note a clear example of the problem and when it began. Bring the medicine list, visit papers, health insurance card, glasses, and hearing aids. Call ahead if your parent needs help hearing, seeing, or using the room, and ask for an interpreter if needed.
At the visit, let your parent answer first. With their okay, add what you have seen, and write down the answers. Before you leave, say the steps for home back to the doctor. Ask when test results should come and who will call, and put that date on the calendar. No call does not mean a test was normal.
