How to use it
- Add the home address, doctor, pharmacy, and two people who can help.
- The U.S. emergency and Poison Help numbers are already there. Add local police and fire numbers for calls that are not an emergency.
- Ask the doctor which number to call after hours.
- Check the numbers, then show your parent and a trusted helper where the page is. Keep private health and insurance details in the care folder, not on the fridge.
Why one list of phone numbers helps
When a question comes up, one list helps you reach the right person fast. Write the home address at the top, with the unit number, city, and ZIP code, and copy the numbers from visit notes and medicine bottles, so nobody is searching for them when time is short.
Ask the clinic for the number to use when it is closed, and write it beside the doctor’s name. Add the pharmacy, a neighbor, a loved one and a backup, and the local police and fire numbers for calls that are not emergencies.
Keep health and insurance details off the fridge; they belong in the private care folder. Share only what a helper needs. In a crisis with danger right now, call 911. Use this list for every other call.
